The medication may be given in the nurse’s clinic. Please complete the Parental Request for Giving Medicine at School and return it to the nurse’s office. The prescribing physician must sign the form for any prescription medication. A new form must be completed with any dosage changes. Parents may bring the medication to the nurse’s office. All prescription medication should be furnished in the original container, labeled with the name of the medicine, the dosage to be given, time(s) of day to be taken, and the expected duration of treatment.
**It is the parent’s responsibility (not the student) to deliver and pick up medications.
**The count of all controlled substances will be documented by the parent and nurse.

Please complete the Parental Request for Giving Medicine at School and return it to the nurse’s office with the parent’s signature; because the medication is over-the-counter, a doctor’s signature is not required. The medication should be in its original container, labeled with the student’s name.

Each student attending St. John’s Christian Academy must have a current South Carolina Certificate of Immunization on file. This form may be obtained from the student’s primary care physician or the Department of Health and Environmental Control (DHEC) office. Records may be delivered to the front office or faxed to 843-899-5514. Please make sure your child’s record is on file before his/her first day of class.
Your child’s physician may document permission to keep an inhaler in a book bag on the Parental Request for Giving Medicine at School.
Students should be without fever (without the use of fever reducing medication) or symptoms of illness for 24 hours before returning to school.
**For example, your child’s last episode of fever was at 6:00 pm on Tuesday evening; he is still considered contagious until 6:00 pm Wednesday evening. So, he may return to school on Thursday morning.
Please do not give your child fever reducing medication (Advil, Motrin, Tylenol) and send them to school. These guidelines help us prevent the spread of illness in the classroom.
A doctor’s note is required in order to be excused from PE class. Excuses may be turned in to the front office, and the instructor will be notified.
